Cancellations and Rescheduling:
It is kindly requested that any cancellation or rescheduling of an appointment is provided at the soonest convenient and reasonable time.
Cancellation or rescheduling within 24 hours of any appointment may incur a charge of the full fee for that appointment, which must be settled by you prior to any future appointments.
In the event of YSIC needing to cancel or reschedule an appointment, there will be no fee to you for the session cancelled by us.
Fees:
Any payment and incurred charges for payment are your liability to pay.

This privacy notice set out is in relation to any session with Yates Sports and Injury Clinic, and your clinician, Dalton Yates, how we collect, use and protect information that is provided by you or collected in relation to your treatment in accordance with the General Data Protection Regulation 2018.

Data Collection, Processing, and Handling:
As a healthcare service to you, there is a requirement to retain information about you, your medical condition, health, personal information, family, employment status, and various other details, provided that this and any other information gathered is relevant to your treatment.
Any changes due to inaccuracies, request to access or delete by you, will be processed as soon as possible and with continued privacy and security.
Due to legal obligations and requirements, not all information is able to be altered or deleted by you upon request.
Information is required to be held for a period of at least 7 (seven) years from the date of the treatment, or, until 7 (seven) years after you have turned 18 (eighteen) years of age if treatment was received prior to this age.
If requested, after this time, information can be disposed of securely.
If there is or is any suspicion of a breach of your personal information, we will inform you as soon as is practically possible about the breach, and any possible impact that this may incur. If a breach occurs that requires us to contact the Information Commissioner’s Office (ICO), this will be done within 3 days of the occurrence, and any additional steps required by the ICO will be completed by us.
To ensure your digital safety, we undertake physical and digital safety precautions to deter any event negatively impacting you.
Access and Sharing of Your Information:
Your information will not be shared with any third party without your consent, with any reasoning for this to be shared prior to this occurring. This may occur in the instance of providing information onto other healthcare professionals, family or for legal requirements.
Your consent will be asked before sharing information, except in any event, accident, or emergency requiring contact to your next of kin or emergency contact provided by you, or medical professionals, or in relation to any court order or relevant legislation.
